How to enroll?
Step 2: Complete the quick online application form.
Step 3: Make the required payment of $75, which includes: $40 (application fee) + $35 (first month of services) as set forth in the Health Care Agreement. The cost is the same if you apply online or at the office. Your payment is processed securely through PayPal.
Step 4: Schedule your appointment online, or if you prefer, directly by phone.
IT IS MANDATORY TO PRESENT THE PROOF OF PAYMENT THE DAY OF YOUR FIRST MEDICAL APPOINTMENT.
